Looking to get rid of paper forms and waivers? League lab has made it easy to upload waivers and forms that are directly integrated into your league site. Before uploading waivers, make sure you have successfully created any custom pages you might need. Once you have a custom page read, follow the steps below to add a waiver.


Step 1. Navigate to you forms (Site tab -> Forms tab). If you are creating a new form select 'Create New Form'. If you are editing a form that has previously been updated, skip to Step 3.



Step 2. Fill out the form details. The 'Name' will be the internal name of the form for your admin to see. The 'Public Heading' will be what is seen on the public site. Make both of these recognizable and easily comprehensible. The 'Submit Button Text' will appear at the bottom of the form once someone has completed it. Make this stand out so that they are aware they need to press the button to submit the form. Lastly, you will want to select how to capture the data and communicate with the individual that their information has been submitted. More than one option can be selected here. Confirm the creation of the form by selecting 'Create the Form'.



Good to know: While there is a button at the end of the form to submission, it happens often where people forget to press the button and reach out asking about their submission (most common places this happens are player waivers as well as staff applications).


Step 3. Adding the Custom Form to a Custom Page. From the custom form page, under the pages column select edit. Select the custom page you want the form to appear on.


Note: Each page can only display one form, so those that already have another form are disabled.
